Include the following information exactly as you submitted it during verification:
First and last name
Email address used to register the account
Phone number on file (if applicable)
A clear photo of the government-issued ID used during your original verification
The photo must be:
In color
Undamaged and readable
Fully visible (no cropping or obstructions)
